All of our appointments are made through our scheduling department via phone.
Your physician has indicated that you are to be referred to our Outpatient Therapy Center or you have decided to take advantage of our Direct Access to therapy.
Our center provides services for physical, occupational and speech therapy.
We appreciate your promptness in arriving 30 minutes prior to your first appointment to assure all information is completed which will allow us to start you at your scheduled appointment time. Our Coldwater patients are required to register at the West Outpatient Registration Desk prior to arriving at our department (located at the West Entrance of Mercer Health off of W. Main Street).
Please bring your insurance cards with you for registration. All minors will need a parent or legal guardian to sign at registration. Your first appointment will be an evaluation lasting typically an hour, which may or may not include treatment as well. If further treatment is necessary, additional appointments will be made after the first visit for the amount of time you and your therapist determine necessary.
Most appointments are scheduled in advance but we will attempt to accommodate your immediate therapy needs.

We try to allow sufficient time for a thorough evaluation and treatment so please give us a 24 hour notice if you need to cancel your scheduled appointment so we can use this time to serve other patients in need of therapy services.

Does my insurance cover therapy?
Nobody wants to get unexpected medical bills. Our staff will contact your insurance company to verify that Community Sports and Therapy Center is in your insurance network. (If Mercer Health is in your network than so are we!) We will also verify which specialty services are covered and the number of approved visits you have per year.

Please note if Mercer Health is in your network, then so is Community Sports and Therapy Center!
The hospital may be listed under varied names with your insurance such as Mercer Health, Mercer County Community Hospital or Mercer Joint Township. All of our billing is completed through Mercer Health located at 800 West Main Street, Coldwater, OH 45828 and the hospital’s tax ID number is 34-1101385. You can use this tax ID number to help verify that the hospital is in your insurance’s network.
Due to our billing being completed through Mercer Health, if you ask your insurance company if Community Sports and Therapy Center is in your network they may incorrectly say “no”, ask them to look up Mercer Health as referenced above.

Information about Insurance Coverage and Billing
Mercer Health provides all the billing for the therapy services provided by the Community Sports and Therapy Center. Mercer Health and the Community Sports and Therapy Center are in network with most insurance companies! If Mercer Health is in your insurance’s network then you are in-network for services provided by the Community Sports and Therapy Center. Networks can be confirmed by searching with the hospital’s Tax ID #341101385.
At Mercer Health, we believe you have a right to know how much you or your insurance company may be charged for your therapy. Prices for therapy services are available upon request and also on Mercer Health’s website. We charge the same for all patients, but a patient’s responsibility may vary, depending on payment plans negotiated with individual health insurers. Uninsured or underinsured patients should consult with our billing staff to determine whether they qualify for discounts.
Payments can also be conveniently made with a credit card, by completing a payment consent or by personal check. Note: Returned checks will result in a $25 fee.

Did you know many insurance companies allow you Direct Access to Physical, Occupational & Speech Therapy?
Direct Access allows skilled therapists to provide specialized therapy without a physician referral. The staff at Community Sports & Therapy Center is able to provide direct care for you, and we will continue to work closely with your primary physician to meet your desired therapy goals.
A majority of insurance companies cover therapy services provided through Direct Access. Many insurance companies are recognizing that Direct Access allows for lower costs and improved access to physical, occupational and speech therapy care. If you have questions about payment for therapy services provided without a referral contact your insurance provider.
**Please note that patients with Medicare insurance are still required to obtain a referral from your physician for therapy.**
